1. Go to http://www.botoxcosmetic.com/seven
2. Fill out the form and make sure you write "seven" (no quotes) in the box labelled "Entry Code"
3. Click submit and you will receive the code on the confirmation page
4. Enter this code on the rebate form given to you by your doctor

Note: Be sure to have pop-ups enabled for the Botox web site since many have reported that the redemption code comes in the form of a web pop-up.
